These terms govern your use of peptide12.com and the membership services Peptide12 provides.

Who we are, and who provides your care

Peptide12 provides non-clinical membership services: patient education, care navigation, scheduling support, and the interface you use to complete your intake and manage your membership. Peptide12 is not a medical practice and not a pharmacy.

Clinical services are provided by OpenLoop Healthcare Partners, PC and its affiliated professional entities, through licensed clinicians who exercise independent professional judgment. Medication is dispensed by the licensed pharmacies listed below. Before treatment begins you will be asked to review and consent to the Telehealth Informed Consent and the patient portal Terms of Use.

Eligibility

You must be at least 18 years old, live in the United States in a state or territory where services are available, and be able to form a binding contract. Some protocols carry their own age or clinical eligibility limits, which a licensed clinician applies during review.

Completing an intake or submitting payment does not guarantee that treatment will be prescribed. A licensed clinician may decline, delay, pause, or change a plan based on your health history, medications, labs, or response to treatment.

State coverage

Currently offering services in all 50 states plus Washington D.C. Some services may not be available in all 50 states or Washington D.C. Subject to change.

Availability depends on the specific product, clinician licensure, patient eligibility, pharmacy access, and state-specific rules. Some states require a synchronous (live video) visit before an initial prescription can be issued.

Pharmacy partners

Peptide12 does not compound, manufacture, dispense, or ship medication, and does not control pharmacy operations. Prescriptions are filled by the following licensed pharmacy partners. Which pharmacy fills a given prescription depends on the medication, your state, and availability.

For questions about your medication, shipment, or membership, contact support@care.peptide12.com rather than contacting a pharmacy directly, so your care team can see and document the request.

Compounded medications

Some medications offered through Peptide12 are compounded preparations, made by a licensed compounding pharmacy under an individualized prescription. Compounded drug products are not FDA approved. The FDA does not evaluate compounded products for safety, effectiveness, or quality. Compounded preparations are not generic equivalents of, or substitutes for, any brand-name product.

Content on peptide12.com, including product pages, guides, and blog posts, is general information and is not medical advice, diagnosis, or treatment, and does not create a clinician-patient relationship. Never disregard or delay seeking professional medical advice because of something you read here. If you think you are having a medical emergency, call 911 or go to the nearest emergency room.

Subscriptions and billing

Membership plans are subscriptions that renew automatically at the interval shown at checkout until cancelled. Payment is collected through the patient interface and processed by our clinical partner, which handles patient billing. Prices are cash-pay; insurance is not billed. HSA and FSA cards are accepted.

Cancellation and refunds

You may cancel your subscription service at any time for any reason, with at least 72 hours' notice before your next billing date. Refunds are limited, and the circumstances in which one is and is not issued are set out in full in our Cancellation & Refund Policy, which forms part of these terms.

Your responsibilities

You agree to provide accurate and complete information during intake and follow-up, including your full medication list, allergies, and medical history, and to keep your account credentials secure. Treatment decisions depend on that information being accurate.

Medication prescribed to you is for your use only. Do not share, resell, or transfer it.

Intellectual property

The content, design, and trademarks on peptide12.com are owned by Peptide12 or its licensors and may not be copied or reused without permission, except for personal, non-commercial use.

Disclaimers and limitation of liability

This website is provided on an "as is" and "as available" basis, without warranties of any kind to the fullest extent permitted by law. To the fullest extent permitted by law, Peptide12 is not liable for indirect, incidental, special, or consequential damages arising out of your use of this website.

Nothing in these terms limits liability for the professional medical services provided by licensed clinicians, which are governed by the patient portal terms and the telehealth consent you agree to before treatment.

Changes to these terms

We may update these terms as our services, pharmacy partners, or state coverage change. The "last updated" date above reflects the most recent change. Continued use of the site after an update means you accept the revised terms.
