Direct answer

Human evidence does not establish that compounded NAD+ injections predictably raise, lower, or normalize blood pressure. A 2026 retrospective pilot of commercially administered intravenous NAD+ measured blood pressure and reported increased heart rate, chest pressure, and gastrointestinal symptoms during some infusions, but it did not establish a blood-pressure treatment effect or a safety rule for subcutaneous or intramuscular injections. Oral nicotinamide riboside and NMN studies use different substances and routes and cannot be converted into an injectable NAD+ protocol. NAD+ is not an established blood-pressure treatment

NAD+ is a coenzyme involved in cellular metabolism, but a biological role does not prove that a commercial injection safely changes blood pressure. A small 2026 IV pilot found tolerability signals, not a home BP rule

A 2026 retrospective pilot reviewed four consecutive days of commercially administered IV NAD+ or IV nicotinamide riboside with 30 days of follow-up. Blood pressure and resting heart rate were among the recorded outcomes. The abstract reported moderate-to-severe gastrointestinal symptoms, increased heart rate, and chest pressure during NAD+ infusions, with symptoms resolving when the infusions ended. This preliminary real-world study was not proof that NAD+ treats blood pressure, and an IV clinic experience cannot be transferred to a compounded subcutaneous or intramuscular injection, nasal spray, or oral supplement.

  • Do not copy an IV infusion amount, duration, symptom response, or monitoring routine into an at-home injection plan.
  • Chest pressure or a fast heart rate during any wellness infusion is not evidence that a product is “working”; it deserves immediate attention from the supervising team.
  • Evidence from one NAD-related form or route does not establish the purity, sterility, absorption, adverse-effect rate, or cardiovascular effect of another product.

Oral precursor studies

NR and NMN blood-pressure findings do not transfer to injectable NAD+

Nicotinamide riboside (NR) and nicotinamide mononucleotide (NMN) are NAD-related precursors, not interchangeable names for compounded NAD+ injection. In a 2025 pilot randomized trial of 54 adults age 55 or older with elevated systolic pressure, oral NR plus supervised walking was not superior to placebo plus walking for the primary daytime systolic-blood-pressure outcome. A 2026 NMN meta-analysis described small preliminary changes in some blood-pressure measures and called for larger, longer trials. These findings do not support using injection dose, timing, or eligibility rules to raise or lower blood pressure.

  • Do not combine NAD+ injection, NR, NMN, niacin, and other “NAD boosters” to chase a blood-pressure result.
  • A supplement result cannot be used to promise the same effect from an injection, infusion, spray, or cream.
